INSURANCES ACCEPTED
All the major PPOs and more!
​We accept all PPO insurances - ranging from major payors such as Aetna, Anthem, Blue Cross, Blue Shield, Cigna, United Healthcare etc.,
​Additionally, we are in-network with Medicare and select regional payers, including Coastal Healthcare, Multiplan, and Health Net.

To confirm your in-network status and understand how copays and deductibles apply to your specific case, please review your insurance plan documents or contact us at payments@lakshmiobgyn.com

We strongly recommend verifying network participation with your insurance provider directly before your visit. For reference, your doctor's Tax ID is 47-2552390. ​

UHC, Cigna, Anthem and Blue Shield, Aetna PPO plans will be billed through the IPA SVMD (Silicon Valley Medical Development). We are contracted through this IPA for the mentioned plans.
For all other plans, we ask you to use the Doctor's TaxID to check with your insurance.
